1. Who we are (APP 1)
Acacia Medical (ACN 660 687 525) is a health service provider bound by the Privacy Act 1988 (Cth).
2. Information we collect (APP 3)
Personal and sensitive health information you provide for telehealth care, including scripts you upload.
3. Notice at collection (APP 5)
We tell you why we collect information before you submit it, and how it will be used and disclosed.
4. Use & disclosure (APP 6)
We use your information to provide care and, with your separate consent, to share scripts with your chosen pharmacy.
5. Cross-border disclosure (APP 8)
Where any service provider stores data outside Australia, we take reasonable steps to ensure APP-consistent handling.
6. My Health Record
Enrolment in My Health Record is optional and never a condition of receiving care.
7. Security & retention (APP 11)
We hold records securely, retain adult records for at least 7 years (until age 25 for minors), and securely destroy them thereafter.
8. Access, correction & complaints (APP 12/13)
You may request access to or correction of your information, and lodge a complaint with us or the OAIC.
9. Data breaches (NDB)
We maintain a data breach response plan and notify affected individuals and the OAIC as required.
